I would like to regiser to attend the Portobello Orchestra rehearsal and/or concert on

I confirm that:

I am not currently being advised to self-isolate;
I have not knowingly come into contact with someone who has COVID-19 or who has been advised to self-isolate over the last 48 hours;
I do not have any of the main symptoms that are listed by the NHS as characteristic of Covid-19 namely:

a high temperature - this means you feel hot to touch on your chest or back (you do not need to measure your temperature);
a new, continuous cough - this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual);
a loss or change to your sense of smell or taste - this means you’ve noticed you cannot smell or taste anything, or things smell or taste different to normal;

The contact details that I provided on my most recent self certification form have not changed;
I agree to inform one of the Portobello Orchestra committee if there is a change to any of the above statements prior to the rehearsal, and to follow the guidance set out in the current Portobello Orchestra COVID-19 Rehearsal risk assessment document when attending the rehearsal.
